社會醫(yī)療保險制度(社會醫(yī)保)是通過個人賬戶和社會
統(tǒng)籌相結合的方式，保障廣大職工的基本醫(yī)療；同時通過醫(yī)
療機構與醫(yī)保管理機構的相互制約，抑制過快的醫(yī)療支出增
長，減少醫(yī)療資源的浪費。社會醫(yī)保的全面實施，是一項復
雜的社會系統(tǒng)工程。涉及到企業(yè)、職工個人、醫(yī)療機構、醫(yī)療
保險局，這些機構和個人在投保、就醫(yī)、補償?shù)榷鄠�環(huán)節(jié)發(fā)生
關系；涉及到大容量信息管理、復雜的計算方法及遠程數(shù)據(jù)
傳遞。如果沒有計算機局域和廣域網(wǎng)絡、沒有適應各個環(huán)節(jié)
的軟件的支持，手工作業(yè)是難以承擔的。醫(yī)院要適應社會醫(yī)
保，就必須對醫(yī)院HIS系統(tǒng)進行改造，才能和醫(yī)保系統(tǒng)接軌。
上海是全國率先實行社會醫(yī)療保險的地區(qū)之一，目前全
國6OO多萬參保人群實現(xiàn)就醫(yī)網(wǎng)上實時結算。上海長征醫(yī)
院是一所擁有近千張床位的三級甲等綜合性醫(yī)院，醫(yī)院IriS
采用“軍字一號”系統(tǒng)，本文主要討論我院HIS“軍字一號”系
統(tǒng)和醫(yī)保系統(tǒng)的接口實現(xiàn)方案。
1 系統(tǒng)數(shù)據(jù)流程
涉及到醫(yī)院的醫(yī)保事務分為三種類型：醫(yī)院與病人的實
時結算；醫(yī)院向醫(yī)保局上報每日收費明細及每月結算報表；
醫(yī)院內部對醫(yī)保事務的管理。三種類型事務之間的關系如
