A Personal Health Record (PHR) is a repository of an individual’s health and wellness
information, a critical and multi-purpose tool to empower and encourage the participation of
individuals in activities contributing to health and well-being. Traditionally, a PHR was a form
or set of forms devised and stored on paper or in a personal computer by the individual or a
family member seeking to keep track of health information. Successful efforts to organize
personal health information on paper media do exist—municipal immunization records, for
example—for limited purposes. More recently the term PHR has been used to refer to a
collection of web-based tools to create lists, diaries and journals, along with various software
applications to assist in organizing health information around the needs of individuals. |
